St. Xavier High School is sponsored by Tri-Health Partners.
All student-athletes are required to have an OHSAA physical on file (valid 1 year from the date performed) by filling out the Physical Form on your St. Xavier FinalForms account by clicking here. Pages 3 and 4 of the form need to be signed by a doctor and turned into the Athletic Department. St X has offered physicals at school as a service done by our sports medicine staff and Tri Health Partners for St. X families. If you are a parent of an incoming freshman (Class of 2020) your Final Forms account wont be active until June 6th. If you have an appointment before then, please click here to download the 2016-2017 OHSAA Physical Form packet. If you have any questions regarding Physicals, feel free to contact Mike Gordon anytime at firstname.lastname@example.org
